EHS Unit #40 Physicals Scheduled June 6

Girls physicals will be held from 5:30-6 p.m. and boys physicals will be held from 6-6:30 p.m. Doors will open at 5:15 p.m.

All athletes must enter the EHS Main Gym lobby and fill out the IHSA physical for prior to taking the physical. The cost of the physicals is $20 and if paid by check, check must be made payable to cash.

Physical forms are available on the IHSA website at the following link: https://www.ihsa.org/documents/sportsMedicine/2022-23/PreParticipationPhysicalForm.pdf. You can go to www.ihsa.org. then click on the resources tab and then click on the download center.

Once you get to the Download Center, the pre-participation physical form is located near the bottom of the page and it can be printed off and filled out at home. Then bring it with you to the athletic physical night.
