Athletic Physicals Set For Effingham Unit #40 Monday

Effingham Community Unit #40 athletic physicals for junior high and high school athletes will be held Monday, June 5 at Effingham High School gym lobby.
The girls' physicals will be held 5:15 p.m. through 6 p.m. and the boys' physicals will run from 5:45 p.m. through 6:30 p.m. The doors will open at 5:15 p.m. to allow time for the girls to begin filling out their physical cards. Doors will be locked at 6:30 p.m.
Due to changes in the IHSA and IESA policies, all physical cards must be signed by a parent. Parents must accompany students to sign off on the physical cards. Students will be ineligible if the physical card does not have a parent signature in all appropriate locations.
The cost of the physical will be $20. Students should enter the building from the gymnasium entrance doors. Pay for the physical with cash, but if you have to pay by check, then make checks payable to cash.
Important information on the physical cards includes questions regarding medicines, allergies and general health history. In addition, there is a parent signature of consent for the IHSA Steroid Testing Policy.
These physicals are for sports only. Kindergarten, sixth-grade and ninth-grade physicals must be done at the familiy doctor's office. Athletic physicals are only good for a period of 395 days from the date of the physical.
