Theresa L. Hoffman, 47, of Effingham, IL passed away on Tuesday, October 6, 2020 at Carle Foundation Hospital in Urbana, IL.
A celebration of life will be held at a later date. Johnson Funeral Home in Effingham is assisting the family.

Theresa was born on January 28, 1973 in Burlington, WI, the daughter of Danny Hoeppner and Debbie Reeder. She married Jeramy Hoffman on January 23, 2015.
Theresa was a loving mother to five children. She had a sweet and kind soul. Theresa was always there to listen. She enjoyed gaming with her husband, doing crafts and was very artistic and creative.
Theresa is survived by her husband, Jeramy Hoffman; children, Jessica, Rebecca, Jessica, Jacob and Sabrina; and siblings, Deanna Cruz, Mody Cruz, David Hoeppner, and Veronica Cruz.
She was preceded in death by her parents.
