Family Care Associates will be holding a drive-thru Moderna COVD vaccine clinic for Family Care Associates on Wednesday, April 28th from 11 a.m. to 1 p.m. 

This must be your 1st vaccine dose and you must be 18 years or older to qualify. Moderna is a 2-dose series. FCA will contact patients to schedule their 2nd dose once it is available. 

Doses are limited. Patients must submit a request for an appointment via the patient portal or by calling the office at 217-342-7000. 

Patients will have no out-of-pocket cost. 

Family Care Associates is located at 1106 North Merchant in Effingham.
