Registration Information for Cowden-Herrick School District

All students in grades K-12 are to complete final registration on either Wednesday, July 31st from 9:00 – 11:00 a.m. and 1:00 – 3:00 p.m. or Thursday, August 1st from 1:00 – 3:00 p.m. and 5:00 – 7:00 p.m. Both registration days will be held in the Cowden cafeteria due to parking lot construction work scheduled for the last week in July at the Herrick Elementary School. Registration forms will be available online July 26th at http://www.cowden-herrick.k12.il.us/forms. Please complete, print and bring to registration.

Book rental fees are due at registration. Book rental fees are $40.00 plus $50.00 for workbooks for grades K-12. Sports fee is $25 per child, per sport or $50.00 cap per family, Driver Education school fee is $100.00. There will also be student accident insurance for those families that are interested. More information can be found on the website at cowden-herrick.k12.il.us. Lunch and breakfast prices: Lunch $3.00 per day and Breakfast $1.50 per day.

Physical examinations are required for all Kindergarten and entrance into 6th and 9th grades. 9th grade physicals must be on a physical form approved by the State of Illinois State Board of Education and on file in the High School office. The immunization portion must be completed and each 9th grade physical must include proof of the following vaccinations: two Varicella, three Hep B, one TDAP, three or more Polio and two MMR. Seniors are required to have the meningitis vaccine.

Students new to the district must also present evidence of a complete physical examination when registering for their classes. Physical forms are available at the Cowden clinic or on the school website:http://www.cowden-herrick.k12.il.us/forms. All student athletes going out for any sport must have their physicals before they can practice. Cindy Rich, FNP, will be giving student and athletic physicals at the Cowden Medical Clinic in Cowden on Friday, July 26th from 1:00 - 3:00 p.m. Cost of physical is $20.00 cash or check only, made payable to the Cowden Medical Clinic.

Dental exams are required for students in K, 2nd, and 6th grades. Those forms are also available at the school office. The State of Illinois requires all kindergarten students to have an eye exam by an Optometrist, those forms are also available in the school office.

Students in K, 1st, 2nd, 3rd, 5th, and 8th grades, as well as all special education students, are required to have a vision and hearing test during the school year. These tests will be given by the Shelby County Health Department at the school and the fee for this service is $8.00. This fee may be paid at registration. (Make check payable to: Cowden-Herrick C.U.S.D. 3A).

The 2018 -2019 Junior High and High School combined yearbooks have arrived. If you have already purchased your book, you may pick it up during registration. If you have not yet purchased your copy, a limited number of copies will be available at $30.00 each.  2019-20 yearbooks may be preordered/prepaid at registration. Yearbooks are $30.00, additional copies for students residing in the same household are $25.00 each.

School Start and End Times: 8:00 a.m. and 3:00 p.m., shuttles leave at 7:30 a.m. and 2:45 p.m.
